- 5 — See: Name 5 things you can see right now. Look carefully — notice shapes, colours, textures, shadows.
- 4 — Touch/Feel: Name 4 things you can physically feel — the chair beneath you, your feet on the floor, the temperature of the air.
- 3 — Hear: Name 3 things you can hear — ambient sounds, distant sounds, your own breath.
- 2 — Smell: Name 2 things you can smell — or recall 2 smells you like.
- 1 — Taste: Name 1 thing you can taste — or recall a favourite taste.
The Neuroscience
Anxiety is a future-oriented state: the mind is processing a threat that has not yet happened and may never happen. The neural centre of this processing is the amygdala, which generates the physiological stress response (cortisol, adrenaline, increased heart rate) in response to perceived threat — regardless of whether the threat is real or imagined. The prefrontal cortex, which is responsible for regulating the amygdala's response, goes partially offline under high stress, which is why rational reassurance ("there is nothing to worry about") is ineffective.
Sensory grounding works by engaging the sensory cortices — the visual cortex, somatosensory cortex, auditory cortex, olfactory bulb, and gustatory cortex — which together represent a large portion of the brain's processing capacity. When these areas are engaged with present-moment input, they draw neural resources away from the amygdala's threat-processing, giving the prefrontal cortex the breathing room it needs to reassert its regulatory function. In very high arousal states, the visual cortex is often partially overwhelmed by threat-scanning.
